About the position

The LCSW Gero Psych position at HonorHealth involves working with patients in a hospital setting to ensure smooth transitions of care. The role focuses on conducting psycho-social assessments, facilitating therapy, and collaborating with healthcare providers and community programs to support patient discharge and ongoing care. This position is integral to creating a multi-dimensional care experience for patients, emphasizing the importance of effective communication and coordination among all members of the healthcare team.

Responsibilities

  • Accountable for an assigned caseload working with patients, healthcare providers, and external partners to ensure a smooth transition of care.
  • Conduct psycho-social assessments and facilitate individual, group, and family therapy as clinically indicated.
  • Collaborate with community-based programs and the behavioral health team to ensure timely discharge with coordination of services and resources upon discharge.
  • Facilitate education activities related to behavioral health and the patient population as requested.
  • Provide oversight of behavioral health interventions, therapy services, and the treatment plan.
  • Assure that appropriate social work or behavioral health interventions are in place to meet effective patient discharge.
  • Lead unit-based rounding and care conference discussions.
  • Serve as a liaison with the care team, community partners, and other members of the patient's inpatient and outpatient care team.
  • Participate in unit-based rounding with the multi-disciplinary team.
  • Facilitate smooth and timely transition from acute care to the appropriate level of care by providing communication of clinical information and plan of care between specialists, PCP, clinic teams, and other key providers.
  • Communicate information pertinent to the discharge plan to the patient, family, MPOA, etc.
  • Assure effective transition and final hand-off to the next appropriate level of care, facility, or case management team.
  • Communicate information regarding inpatient stay and discharge plans to payer as indicated.
  • Promote a collaborative process and communication between all healthcare team members, inclusive of patients/clients, families, and significant others to ensure integrated care services are targeted, appropriate, and beneficial to the population served from admission through the discharge process.
  • Participate in the development and maintenance of unit metrics.
  • Act as a patient advocate through the continuum and be available to the physician, patient, and family as a resource to facilitate communication and monitor patient care to ensure quality care through the use of standards of care and evidence-based practice guidelines.
  • Advocate utilizing knowledge of applicable laws, regulations, government and insurance benefits as well as practice guidelines and standards of practice.
  • Perform other related duties as assigned or requested.

Requirements

  • Master's Degree in Social Work required.
  • Licensed Medical Social Worker (LCSW) State Licensure required.
  • Minimum of 2 years of hospital experience preferred.
