The position operates Monday through Friday from 8:00 AM to 4:30 PM, providing a structured work schedule that allows for a healthy work-life balance. As an LCSW, you will be required to have a Master of Social Work (MSW) and a valid LCSW license. While prior social work experience is preferred, new LCSWs are also considered for this role. Missouri Baptist Medical Center is an acute care hospital located in St. Louis County, offering a comprehensive range of medical and surgical services. In this role, you will be part of the Care Coordination Department, which includes RN Case Managers, Social Workers, and skilled administrative staff. The Case Management process is a collaborative practice model that involves patients, nurses, social workers, physicians, and caregivers, ensuring effective communication and resource coordination throughout the continuum of care. Your primary responsibility will be to provide clinical social work services that address the psychosocial needs of patients and their families, including individual and group counseling, community resource linkage, and advocacy. You will work closely with the healthcare team to ensure timely and cost-effective discharge and transition planning, ultimately aiming to achieve optimal health outcomes for patients.

Responsibilities

  • Conduct screening and assessment activities to meet the needs of discharged patients.
  • Prepare patient-centered plans of care based on assessment findings.
  • Apply and comply with all regulations while utilizing available resources to manage patient care.
  • Provide individual and group counseling to address psychosocial needs.
  • Link patients and families to community resources and advocate for their needs.
  • Collaborate with the healthcare team to ensure timely discharge and transition planning.

Requirements

  • Master's Degree in Social Work required.
  • Licensed Clinical Social Worker (LCSW) certification required.
  • No prior experience required; new LCSWs considered.
  • Experience in case management and clinical social work preferred.
  • 2-5 years of experience in a related field preferred.
