In this role, you will be responsible for conducting patient assessments to develop individualized behavior intervention plans tailored to each client's needs. You will implement and monitor these plans to improve patient outcomes, collaborating with interdisciplinary teams to provide comprehensive care. Maintaining accurate and confidential patient records in compliance with HIPAA regulations is essential, as is providing training and support to caregivers and other professionals involved in patient care.
