About the position

The Lead Care Manager at Home Matters Caregiving is responsible for providing leadership and support to caregivers in the home care environment. This role involves conducting client assessments, developing personalized care plans, and ensuring high-quality service delivery. The Lead Care Manager acts as a mentor and coach, guiding caregivers from orientation through their initial shifts and client transitions, while also covering shifts as needed.

Responsibilities

  • Conduct initial assessments of clients referred for non-medical in-home care services.
  • Develop comprehensive care plans tailored to each client's specific needs and preferences.
  • Perform in-person client reassessments as scheduled and notify the main office of any changes in the client's condition that may necessitate changes to the established care plan.
  • Regularly review and update care plans as needed to ensure the highest quality of care.
  • Lead a team of caregivers to provide guidance and support.
  • Maintain accurate client records and documentation.
  • Collaborate with the administrative team to optimize staffing efficiency.
  • Conduct orientation, onboarding, and training for new caregivers as needed.
  • Participate in daily staff and team meetings.
  • Cover open caregiving shifts in the event of call outs or availability.

Requirements

  • Mid-level experience in home health care (3-5 years preferred).
  • Proficient in Microsoft Office.
  • Valid driver's license and automobile insurance compliant with company policies.
  • Strong organizational and time-management skills.
  • Excellent active listening skills and a strong service orientation.
  • Good interpersonal, written, and verbal communication skills.

Nice-to-haves

  • CNA or LPN certification helpful but not required.
  • Experience using Wellsky is a plus.
