About the position

The Lead Case Manager at Special Service for Groups, Inc. is responsible for providing intensive case management services to clients who are homeless or chronically homeless. This role involves assessing client needs, coordinating services, and developing individualized intervention plans to help clients transition to permanent housing. The Lead Case Manager will work closely with clients to address their immediate needs and facilitate access to various resources and services.

Responsibilities

  • Screen referrals to determine eligibility and coordinate client intakes and assessments.
  • Assess and address emergent needs of clients including medical care, shelter, food, and clothing.
  • Evaluate client needs regarding chemical dependency, mental health, medical care, housing, and employment.
  • Assist clients in identifying appropriate housing and maintaining occupancy.
  • Develop and implement Individual Intervention Plans with client input.
  • Update service plans quarterly or as needed to reflect client progress and emerging needs.
  • Provide post-discharge information and referrals for homeless prevention services.
  • Enter all data into the EHR system within 24-48 hours of client contact.
  • Provide structured Intensive Case Management Services and advocacy according to SSG/HOPICS policies.
  • Assist clients in scheduling and keeping appointments.
  • Facilitate referral and linkage processes to appropriate services.
  • Develop a network of housing resources and assist clients in accessing housing.
  • Identify gaps and barriers in community resources and advocate for client accessibility.
  • Attend Case Management staff meetings and other required meetings.
  • Maintain client files and reporting for assigned caseload according to contract requirements.
  • Effectively manage assigned caseload and reporting requirements.
  • Provide a range of supportive services including housing assistance, health linkages, and crisis management.
  • Participate in meetings with collaborative agencies and community meetings as required.
  • Administer and screen all cases using various screening tools.
  • Complete non-clinical intake documents and update individual service plans regularly.
  • Conduct 90% of services in the field and 10% in the office.
  • Monitor and follow up with clients on an ongoing basis.

Requirements

  • Bachelor's degree from an accredited university or two years of experience in the social service field.
  • Minimum of three years drug and alcohol-free if in recovery.
  • Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds.
  • Effective communication skills, both written and orally.
  • Valid driver's license and reliable transportation.
  • Current proof of auto insurance required.
  • TB test required within specified time frames.
  • CPR and First Aid Certification required within 30 days of employment.
  • Basic computer word processing skills.
