About the position

The LEAD Case Manager at Baltimore Crisis Response, INC is responsible for supporting and empowering participants of the Law Enforcement Assisted Diversion (LEAD) program. This role focuses on helping individuals access community resources to enhance their recovery, health, and overall well-being, while preventing their entry or re-entry into the criminal justice system, psychiatric hospitalization, or homelessness. The position requires collaboration with law enforcement and community partners to ensure effective integration of participants into the community.

Responsibilities

• Provide services in the least restrictive environment, meeting LEAD participants in their homes or other community settings.
• Maintain a case load of LEAD participant clients and provide specialized intensive case management services as needed.
• Work closely with Police partners who are the primary referral source for LEAD.
• Provide linkages to care and other community resources to assist participants in maintaining recovery.
• Collaborate with the LEAD Peer Recovery Specialist to help participants navigate barriers to health and behavioral health care.
• Maintain a flexible schedule from 10 am to 6 pm, adjusting hours as necessary based on participant needs.
• Maintain collaborative relationships with community partners and support service agencies.
• Provide transportation to participants for appointments when necessary.
• Complete timely and accurate documentation and outcome tracking forms for caseload participants.
• Attend weekly operational workgroup meetings to discuss participant progress.
• Attend trainings and conferences to improve program skills.

Requirements

• Bachelor's degree in social work, psychology, or a related human service field from an accredited college or university.
• Experience working with individuals with mental illness, addiction, and/or co-occurring disorders is preferred.
• Valid MD driver's license.
