About the position

The Lead Clinical Pharmacist at Dartmouth Health plays a crucial role in assisting the Pharmacy Manager with the establishment of policies, procedures, and standards for pharmacy services. This position ensures compliance with quality standards and manages clinical pharmacy services while performing essential pharmacist duties as needed.

Responsibilities

  • Assist the Pharmacy Manager in establishing policies and procedures for pharmacy services.
  • Perform central and decentral pharmacist staffing responsibilities as needed.
  • Evaluate medication orders for appropriate dosing and drug interactions.
  • Conduct therapeutic drug monitoring for specific medications.
  • Adjust medication dosages based on patient renal function.
  • Participate in multidisciplinary rounds for patient care optimization.
  • Conduct medication reconciliation for admitted patients.
  • Evaluate chemotherapy regimens for dosing accuracy and supportive care.
  • Verify medication orders for chemotherapy and supportive care medications.
  • Prepare and evaluate compounded sterile preparations according to USP guidelines.
  • Serve as a resource for providers, nurses, and patients regarding medication use.
  • Support the pharmacy manager in clinical pharmacy programs and quality improvement initiatives.
  • Participate in medication-related committees to meet departmental goals.
  • Supervise and assist pharmacy technicians and students in workflows.
  • Assist with medication distribution and automated dispensing cabinet maintenance.
  • Support management of pharmacy inventory and place replenishment orders as needed.
  • Perform technician duties when necessary.
  • Assist the manager in developing policies and procedures.
  • Ensure compliance with regulatory requirements and proper operation of pharmacy equipment.
  • Direct pharmacy operations in the manager's absence.

Requirements

  • 1 to 3 years of experience required.
  • Hospital pharmacy experience preferred.
  • Graduate of an ACPE accredited School of Pharmacy.
  • Current NH pharmacist licensure required.
  • Ability to work independently with minimal supervision.
  • Effective oral and written communication skills.
  • Strong interpersonal and team skills.
