About the position

Everyday Life Consulting is seeking an experienced Lead Community Health Worker (CHW) to manage and support operations at Neighborhood Wellness Centers. This role focuses on promoting health equity, coordinating community outreach, and mentoring CHWs to ensure high-quality service delivery. The Lead CHW will engage with diverse populations and foster community wellness through effective leadership and collaboration.

Responsibilities

  • Manage the day-to-day CHW operations at one or more Neighborhood Wellness Centers.
  • Support training and supervision of health-related services provided by Community Health Workers.
  • Oversee the implementation of organizational policies and procedures at each assigned NWC location.
  • Ensure thorough documentation of service provision in the Electronic Health Records (EHR) system.
  • Coordinate CHW-led health services with facility management and testing/diagnostic screening partners.
  • Collaborate with project management and regional managers to maximize service delivery across NWC locations.
  • Partner with local and regional constituents to promote and enhance the work of the NWCs.

Requirements

  • Bachelor's degree and a minimum of three years of related professional experience.
  • Community Health Worker certification (current or lapsed) and experience conducting work commonly executed by Community Health Workers.
  • Work experience in healthcare, managed care, and/or public/community health-related settings.
  • Experience with staff management including hiring, onboarding, training, mentoring, supervising, and evaluating.
  • Experience in project management, preferably in healthcare, managed care, or public/community health settings.
  • Effective communication and interpersonal skills, with the ability to work collaboratively in team settings.
  • Experience successfully working in diverse communities and with diverse groups of individuals.
  • Strong technology skills including Microsoft Office 360 and virtual communication tools.
