Lead LTSS Care Manager (RN)

About the position

The position involves developing, assessing, and coordinating holistic care management activities for high-risk or high-acuity populations, focusing on medical and physical health needs. The role requires frequent travel to visit members, performing assessments, creating personalized care plans, and acting as a liaison between members, families, and healthcare providers to ensure quality and cost-effective healthcare outcomes.

Responsibilities

• Develop and coordinate holistic care management activities for high-risk populations.
• Perform assessments and develop personalized care/service plans for members.
• Educate members, families, and caregivers on available services and benefits.
• Evaluate medical, behavioral, and social needs of members and recommend care plans.
• Complete assessments related to medical/physical health needs and general health status.
• Act as a liaison and advocate for members between families, physicians, and agencies.
• Monitor member status for complications and clinical symptoms, adjusting care plans as necessary.
• Ensure appropriate referrals based on individual member needs and facilitate access to services.
• Provide education on preventive care and treatment options to members and caregivers.
• Maintain accurate documentation in the electronic care management system and ensure compliance with guidelines.

Requirements

• Graduate from an Accredited School of Nursing or a Bachelor's degree in a related field.
• 2 - 4 years of related experience in care management or nursing.
• RN - Registered Nurse State Licensure or NP - Nurse Practitioner State Licensure required.
