About the position

The Bilingual Lead Medical Assistant, also known as the Clinical Care Coordinator (CCC), plays a crucial role in coordinating clinical care activities for assigned care teams. Reporting to the Clinical Support Manager, the CCC serves as the clinical lead, providing essential support to clinical providers and other team members. This position focuses on personalized disease management, patient education, and ensuring continuity of care with external healthcare organizations. The CCC also participates in quality improvement initiatives and assists with direct patient care as needed.

Responsibilities

• Coordinate clinical care activities for assigned care teams.
• Provide clinical support for medical assistants and other team members.
• Manage personalized disease management services for patients.
• Coordinate continuity of patient care with external healthcare organizations.
• Provide patient education and health behavior coaching.
• Assist with quality improvement audits and initiatives.
• Administer immunizations and medications as directed by the provider.
• Facilitate communication among care team members regarding patient care plans.
• Conduct individual and group patient education sessions.
• Track lab results and referrals for care teams.

Requirements

• Bilingual in Spanish and English is required.
• High school diploma or equivalent is required.
• Formal Medical Assistant training is preferred.
• 2-5 years of experience in a clinical setting is required.
• Experience with EMR systems is required (1 year).
• Experience in taking vital signs is required (1 year).
• BLS Certification is required.
• Certified Medical Assistant certification is required.

Nice-to-haves
