About The Position
The Bilingual Lead Medical Assistant, also known as the Clinical Care Coordinator (CCC), plays a crucial role in coordinating clinical care activities for assigned care teams. Reporting to the Clinical Support Manager, the CCC serves as the clinical lead, providing essential support to clinical providers and other team members. This position focuses on personalized disease management, patient education, and ensuring continuity of care with external healthcare organizations. The CCC also participates in quality improvement initiatives and assists with direct patient care as needed.
