About the position
The Lead RN Case Manager at Holy Cross Health is responsible for overseeing daily operations within the care management department, providing education and support for new staff, and ensuring continuous improvement in documentation practices. This role involves guiding a team in resource management and advocating for patients' healthcare needs, all while maintaining high standards of quality outcomes.
Responsibilities
- Assist with daily operations within the care management department.
- Provide process oversight and education for new staff orientation.
- Support staff competency development and continuous documentation improvement program (CDIP).
- Assist leadership with employee supervision, management, and training.
- Conduct data research and analysis.
- Guide the team in resource management.
- Maintain a collaborative process of assessment, planning, facilitation, and advocacy for patients.
- Individualize services to meet patients' healthcare needs using available resources.
Requirements
- At least two years of case management experience, including Quality Improvement and Clinical Documentation enhancement in an acute hospital setting.
- Current licensure by the Maryland State Board of Nursing or Compact State as a Registered Nurse.
- Graduation from an accredited nursing program; Associate Nursing Degree (BSN or MSN) preferred.
Nice-to-haves
