About the position

The Lead Social Worker - LCSW - Inpatient Case Manager is responsible for overseeing the case management staff, ensuring core competencies are maintained, and participating in training and process improvement initiatives. This role involves mentoring new employees, managing caseloads, and collaborating with leadership to enhance case management practices. The position requires strong communication and organizational skills, as well as the ability to lead performance improvement initiatives and ensure staff accountability.

Responsibilities

  • Assist Management and the Educator in developing and maintaining core competency of the Case Manager staff.
  • Participate in department curriculum and training in collaboration with the Educator.
  • Assist with department process improvement initiatives.
  • Develop and mentor case management staff.
  • Collaborate with leadership and departmental staff regarding changes impacting case management's role.
  • Lead performance improvement initiatives and provide input on staff accountability.
  • Ensure staff coverage by maintaining staffing schedules and time off calendars.
  • Monitor front line staff data for performance and accountability.
  • Cross cover Case Manager positions across the care continuum as needed.
  • Attend select meetings and represent the Case Management Department.

Requirements

  • Masters Degree in Social Work.
  • A minimum of five years acute care case management experience required.
  • CCM (Case Management Certification) or ACM (American Case Management Certification) required within 2 years of date of hire.
  • 12 months full-time case management experience supervised by a certified case manager, or 24 months full-time case management experience not supervised by a certified case manager.

  • Strong facilitation, training and presentation skills.
  • Ability to work successfully and efficiently both independently and with others.
  • Expertise in computer applications including Microsoft Office, Adobe and MCCM.
  • Excellent organizational skills with strong ability to multi-task effectively and efficiently.
  • Knowledge of key case management functions and regulatory requirements by State and Federal governments.
