ESSENTIAL FUNCTIONS SUMMARY: The Lead Young Adult Case Manager reporting to the YV LifeSet Team Supervisor will offer support to the full team, as well as, to the YV LifeSet Team Supervisors. The Lead Young Adult Case Manager will maintain a reduced caseload and will provide services to address the needs of their assigned young people. This individual support includes skill development, obtaining and maintaining safe and suitable housing, meeting educational goals, obtaining and maintaining employment, building a support system and remaining free from legal involvement. The position will also offer support in training, mentorship, coverage, crisis support, data collection and analysis and other areas of need. ESSENTIAL FUNCTIONS & RESPONSIBILITIES: Provides individual services to a reduced caseload of young adults in the home and community to assist with the transition to adulthood, meeting face to face at the frequency of sessions determined by the identified level of care and provides additional support via phone, text, or in person as needed. When needed, daily contact may be necessary to ensure safety and stability. Responsible for all interventions associated with their assigned young adults. This includes case management and collaboration with external key stakeholders and referral sources, such as probation, mentors, therapists, etc. Provide training and guidance to new hires or as needed to current staff-to include initial and ongoing training, observation, scheduled office hours, daily check ins, etc. Provide assistance to Supervisor to ensure compliance and quality of services and documentation-to include mock reviews, spot checks, surveys and other quality assurance activities. Facilitate, attend and/or engage in marketing activities to promote local program awareness to recruit program referrals and staff, when needed. Provide leadership support Review Treatment Plans, Conduct Group Supervision, Facilitate Clinical Consultation in the absence of the LifeSet Supervisor due to illness, vacation, etc. Provide case management support activities Session, court, meeting, documentation coverage in the absence of the LifeSet Specialist due to illness, vacation or otherwise assigned by the Supervisor. Conduct ongoing assessment of the young person's needs from a strength focused solution-based perspective. Complete documentation with quality and within the required timeframe. Develop service plans as required by DCFS; attends Administrative Case Reviews, court hearings, IEP meetings and other reviews and staffings, as required. Attend Group Supervision and Clinical Consultation and follow up on and implement feedback and recommendations received from the Supervisor, Program Lead and Licensed Program Expert. Attend all other program required meetings and trainings. Participate in the on-call rotation to provide support to young people after hours and on weekends. Conducts detailed review of youth's case record to identify parents, siblings, foster parents and other significant adults from the child's present and past. Conducts assessments of their capacity and willingness to become or remain a resource to the young person. Establishes a visiting plan and coordinates its implementation. Provides referral and linkage to appropriate resources; secures all written agreements in accordance with guidelines established by contracting agencies and other outside sources. Recommends and obtains Supervisor approval of limited financial assistance for young person's needs. Adhere to all ethical and legal guidelines, including confidentiality and mandated reporting. Performs other related tasks as required or assigned. Every employee must successfully pass Therapeutic Crisis Intervention (TCI) training in all testing portions applicable to their position. Following completion of the full course, staff members must recertify every six months thereafter. Not only must you successfully complete the TCI training, but you must then put that training into practice in your everyday work. Other duties as assigned if applicable. QUALIFICATIONS & SKILLS: Master's degree in field of social work, psychology, or related field preferred. Bachelor's degree in social services with two years related experience in counseling and/or case management is required. Licensure by DCFS or eligible for licensing. Create organized plans so that all casework tasks can be completed as required. Respond in a timely manner to all written correspondence, email, and phone call inquiries regarding young people. Contact courts, DCFS, and other service providers or community organizations to coordinate services or to make referrals. Maintain accurate, complete, and weekly records and data. Demonstrated understanding of trauma-informed practice. Working knowledge of community resources to use in serving young people and maintains productive working relationship with these resources. Ability to manage flexible work schedule, including evenings and weekends. Proficiency in operating a computer with Microsoft Office suite of applications. Ability to interpret and accurately apply the regulations of federal and state programs. Respect for cultural and socioeconomic characteristics of clients and coworkers.
