About the position

The Level 2 Post Release Services (PRS) Case Manager plays a critical role in providing essential support and services to unaccompanied children and their sponsors. This position requires a high level of technical and professional expertise to ensure the safety and well-being of children, focusing on their successful integration into new environments.

Responsibilities

  • Provide culturally and linguistically appropriate post placement services and comprehensive case management for unaccompanied children (UC).
  • Carry a minimum caseload of 15 PRS cases, providing services virtually and in person every 90 days.
  • Complete individualized needs assessments with UC and sponsors, developing case plans with attention to culture, language, and special circumstances.
  • Provide ongoing case management services based on assessed needs.
  • Maintain high standards of record-keeping according to policies and procedures utilizing the UC Portal and internal software systems.
  • Provide information, education, referrals, outreach, advocacy, and support to ensure children receive necessary services.
  • Assess the safety and appropriateness of placements using child welfare principles and social work best practices through various communication methods.
  • Maintain confidentiality of health care records and information in compliance with HIPAA and PHI requirements.
  • Coordinate and maintain communication with key stakeholders.
  • Participate in regular training and communication sessions using platforms like Teams and Zoom.
  • Display professional conduct and ensure respectful interactions with all contacts.
  • Adhere to all state and federal laws and regulations, and follow company policies and procedures.

Requirements

  • A bachelor's degree in social work, behavioral science, or social services with at least two years of progressive experience in social services, case management, or child welfare.
  • Experience in child welfare, child placements, family preservation, and social service provision is desired.
  • Skilled at writing professional, thorough assessment reports.
  • Extensive knowledge of local resources for immigrant communities is desired.
  • Excellent organizational, verbal, written, and interpersonal communication skills, with the ability to prioritize duties in a fast-paced environment.
  • Must have a valid driver's license, reliable transportation, insurance, and a clean driving record.
  • Position requires up to 50% long-distance travel with limited notice.
  • Proficient in Microsoft Office, including Excel and Teams, for maintaining regular contact with supervisors.
  • Ability to type and be computer literate, comfortable using a tablet/iPad or laptop for daily assignments.
  • Fluency in Spanish is preferred, with proficiency in additional languages being a plus.
