About the position

The Licensed Behavioral Health Clinician at Hartford HealthCare plays a crucial role in delivering evidence-based clinical services within the Outpatient department, focusing on the Young Adult Partial Hospitalization Treatment and Intensive Outpatient Treatment programs. This position involves conducting assessments, developing care plans, and providing direct patient care, including individual, group, and family therapy, while maintaining ethical standards and professional practices.

Responsibilities

  • Conduct comprehensive bio-psychosocial assessments and establish care plans as appropriate.
  • Provide direct patient care including case management, care coordination, crisis intervention, counseling, and emotional support.
  • Refer patients to appropriate levels of care and resources that promote individualized care plans.
  • Maintain timely patient-centered documentation reflecting pertinent information for assessment, interventions, and outcomes.
  • Independently provide clinical social work interventions to all patient populations.
  • Provide social work field supervision to second-year social work students.

Requirements

  • Master's degree in social work, counseling, marriage and family therapy, or related Human Service field.
  • Previous experience in a healthcare setting, preferably with a chemical dependency population, adults preferred.
  • Current Connecticut license; LCSW, LPC, LMFT, LADC, or Psychologist.
  • Excellent communication skills with the ability to establish relationships within and outside the entity.
  • Cognizant of health-related social needs and cultural competence, recognizing and addressing health disparities.

Nice-to-haves

  • Experience working with young adults in a behavioral health setting.
  • Knowledge of community resources for mental health and substance abuse treatment.
