About the position

The Licensed Master Social Worker (LMSW) position involves working as part of a multidisciplinary treatment team to provide comprehensive care and support to individuals facing various mental, emotional, and behavioral challenges. The role includes assessing individual needs, developing treatment plans, advocating for clients' rights, and ensuring continuity of care through collaboration with community providers. The LMSW will also document progress and may provide clinical services under supervision.

Responsibilities

• Serve as a member of a multidisciplinary treatment team.
• Participate in team meetings and present clinical information and observations.
• Provide consultation and assistance to staff in other disciplines.
• Coordinate care and treatment planning for individuals.
• Assess and evaluate individual needs, strengths, and limitations.
• Record significant psychosocial events and changes in clinical conditions.
• Advocate for individuals' rights and access to benefits and services.
• Help individuals identify goals and solutions to various conditions.
• Use therapy to address mental illnesses and emotional disturbances.
• Establish and maintain professional relationships with community providers.
• Assist residents with the development and maintenance of natural supports.
• Link residents and families to existing community services.
• Develop effective treatment plans in conjunction with residents and treatment teams.
• Monitor and evaluate residents' progress and modify treatment plans as needed.
• Determine discharge service needs and appropriate community resources.
• Prepare and maintain documentation and records, including progress notes and discharge plans.

Requirements

• Possession of a license and current registration as a Licensed Master Social Worker in New York State.

Benefits

• Annual salary range from $72,032 to $91,381
• Additional $3,400 annual location pay
