The Licensed Master Social Worker (LMSW) - Homecare position at Hartford HealthCare involves providing personalized care to patients in their homes, focusing on their bio-psychosocial needs. The role requires collaboration with patients, families, and healthcare teams to develop and implement care plans that address both short and long-term needs. The LMSW will utilize their clinical skills to conduct assessments, provide counseling, and advocate for patients, ensuring they receive the necessary support and resources.

Responsibilities

  • Collaborates with the interdisciplinary team regarding patients identified for Social Work Services.
  • Completes comprehensive in-home psychosocial evaluations.
  • Utilizes evidence-based assessment tools to determine cognitive status and behavioral health needs.
  • Determines appropriate level of care including needs for long/short-term skilled-nursing facilities.
  • Creates a care plan with measurable goals based on comprehensive assessments.
  • Implements short-term interventions to address identified barriers and promote health and well-being.
  • Provides clinical treatment services including triage, crisis intervention, and therapy.
  • Utilizes Motivational Interviewing and Coaching skills to assist patients and families in identifying goals.
  • Acts as educators for patients, families, and the community regarding health maintenance and treatment adherence.
  • Advocates for the needs and interests of patients to improve access to care.
  • Monitors and alters care plan goals and interventions as necessary.
  • Maintains timely documentation of social work services reflecting patient information and outcomes.
  • Facilitates team-based care and collaboration with healthcare team members.
  • Participates in case conferences and interdisciplinary team meetings.
  • Identifies community-based resources for patients and families as appropriate.

Requirements

  • 2-3 years of healthcare experience, including acute and community health experience.
  • Licensed Master Social Worker (LMSW) certification.
  • Considerable knowledge of social, cultural, economic, political, medical, and psychological issues influencing patient behavior.
  • Familiarity with evidence-based assessment tools such as PHQ-9, SLUMS, and others.
  • Proven ability to work effectively both independently and in a team environment.
  • Excellent written and verbal communication skills.
  • Strong organization skills and attention to detail.
  • Strong computer skills, including Microsoft Office; Epic experience is a plus.
