About the position
The Licensed Master Social Worker (LMSW) plays a crucial role within the interdisciplinary team, focusing on assessing and addressing the psychosocial needs of patients and their families. This position involves developing psychosocial outcomes, providing psychoeducation, supportive counseling, and casework interventions to enhance the quality of care. The LMSW also engages in team meetings, case planning, and offers bereavement support, ensuring comprehensive psychosocial care is delivered.
Responsibilities
- Perform assessments of psychosocial symptoms of patients and families.
- Develop psychosocial outcomes to alleviate identified symptoms.
- Provide psychoeducation and supportive counseling to patients and families.
- Maintain accurate and timely documentation in patient medical records.
- Participate in interdisciplinary team meetings to plan patient care.
- Refer patients and families to appropriate community resources.
- Provide bereavement care and facilitate bereavement group activities.
- Participate in psychosocial supervision meetings and training sessions.
- May supervise social work interns under the direction of the team manager.
Requirements
- Masters of Social Work from an accredited institution.
- At least one year of social work experience in a healthcare setting.
- Knowledge of psychosocial issues prevalent with terminally ill patients and their families.
- Excellent listening and communication skills.
- Ability to work effectively as part of an interdisciplinary team.
