About the position
The Licensed Master Social Worker (LMSW) plays a crucial role within the interdisciplinary team, focusing on assessing and addressing the psychosocial needs of patients and their families. This position involves developing psychosocial outcomes, providing psychoeducation, supportive counseling, and casework interventions to enhance the quality of care for terminally ill patients. The LMSW also engages in team collaboration, documentation, and may supervise interns, ensuring comprehensive psychosocial support is delivered effectively.
Responsibilities
- Perform assessments of psychosocial symptoms of patients and families.
- Develop psychosocial outcomes to alleviate identified symptoms.
- Provide psychoeducation and supportive counseling to patients and families.
- Maintain accurate and timely documentation in the patient medical record.
- Participate in the preparation and execution of the interdisciplinary plan of care.
- Provide psychosocial education and consultation to the interdisciplinary group (IDG).
- Participate in psychosocial supervision meetings and training sessions.
- Provide bereavement care and facilitate bereavement group activities.
- Act as a coordinator for volunteer interventions on the plan of care.
Requirements
- Master's degree in Social Work from an accredited institution.
- At least one year of social work experience in a healthcare setting.
- Knowledge of community resources and psychosocial issues related to terminally ill patients.
- Excellent listening and communication skills.
- Ability to advocate effectively for patients and families.
