About the position

The Licensed Master Social Worker (LMSW) plays a crucial role in the interdisciplinary team, focusing on assessing psychosocial symptoms of patients and their families. The position involves developing psychosocial outcomes, providing psychoeducation, supportive counseling, and casework interventions to alleviate identified symptoms. The LMSW also participates in the interdisciplinary plan of care, ensuring high-quality psychosocial care and continuity for patients and families.

Responsibilities

  • Perform bio-psycho-social-spiritual assessments of patients and families.
  • Provide individual and family supportive counseling.
  • Refer patients and families to appropriate community resources.
  • Maintain accurate and timely documentation in the patient medical record.
  • Attend weekly team meetings to plan care.
  • Participate in patient case conferences.
  • Provide psychosocial education and consultation to the core interdisciplinary group (IDG).
  • Participate in psychosocial supervision meetings and other trainings.
  • May supervise other psychosocial team members, including social work interns, at the direction of the team manager.
  • Act as the coordinator and communicator of volunteer interventions on the plan of care.
  • Provide bereavement care, including bereavement group activities.
  • Participate in psychosocial spiritual on-call rotation/schedule as assigned.

Requirements

  • Masters of Social Work required from a school accredited by the Council on Social Work Education.
  • At least one year of social work experience in a health care setting.
  • Experience as a social worker with terminally ill patients and their families is preferred.
  • License and/or Certification as required by State Minimum State Requirements.

Nice-to-haves

  • Thorough knowledge of community resources or the desire/ability to obtain this knowledge.
  • Knowledge of key psychosocial issues prevalent with terminally ill patients and their families/caregivers.
  • Capacity to relate and work well with others as a member of an interdisciplinary team.
  • Excellent listening and communication skills.
  • Ability to be an effective patient and family advocate.
