About the position
The Licensed Master Social Worker (LMSW) position at the Central New York Psychiatric Center involves providing professional social work services to individuals with mental illness within a multidisciplinary team. The role focuses on delivering culturally competent and evidence-based treatment in a recovery-oriented environment, ensuring continuity of care through community linkages and discharge planning.
Responsibilities
- Providing crisis intervention, service linkages, and discharge planning.
- Developing treatment plans with individuals and the treatment team; monitoring and evaluating progress; and modifying treatment plans as needed.
- Determining discharge needs and appropriate community resources.
- Establishing and maintaining relationships with community providers to ensure continuity of care.
Requirements
- Must be licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW).
- Ability to provide culturally competent and evidence-based treatment and support services.
