About the position
As a Licensed Master Social Worker, you will be part of a multidisciplinary team that provides culturally competent and evidence-based treatment and support services in a recovery-oriented environment to individuals with mental illness. Your role will involve providing crisis intervention, service linkages, and discharge planning, as well as developing treatment plans with individuals and the treatment team, monitoring and evaluating progress, and modifying treatment plans as needed. You will also determine discharge needs and appropriate community resources, and establish and maintain relationships with community providers to ensure continuity of care.
Responsibilities

Requirements
- Licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW)
