Responsibilities
- Conduct comprehensive assessments to identify barriers hindering individuals from achieving independence and community integration.
- Complete specialized evaluations related to various risk factors and rights issues.
- Develop and execute personalized Treatment Plans to empower individuals to overcome obstacles and actively participate in their community.
- Regularly review progress and collaborate with the DHS Treatment Coordinator to ensure the effectiveness of interventions.
Requirements
- Master's Degree from an accredited school of Social Work.
- NYS Licensure as a Licensed Master Social Worker.
- Preference for candidates with licensure as a Licensed Clinical Social Worker.
- Previous experience in a human service setting, specifically supporting individuals with developmental disabilities.
- Strong communication skills, empathy, and problem-solving abilities.
