Responsibilities

• Conduct comprehensive assessments to identify barriers to independence and community integration.
• Complete specialized evaluations related to various risk factors and rights issues.
• Develop and execute personalized Treatment Plans for individuals.
• Regularly review progress and collaborate with the DHS Treatment Coordinator to ensure effective interventions.

Requirements

• Master's Degree from an accredited school of Social Work.
• NYS Licensure as a Licensed Master Social Worker.
• Preference for candidates with licensure as a Licensed Clinical Social Worker.
• Previous experience in a human service setting, particularly with individuals with developmental disabilities.
• Strong communication skills and empathy.
• Problem-solving abilities and a customer-centric approach.
