About the position
The Licensed Master Social Worker (LMSW) plays a crucial role in providing psychological support and resources to residents at the NYS Veterans Home at Montrose. This position involves assessing and evaluating residents' needs, formulating care plans, and collaborating with interdisciplinary teams to ensure optimal care for veterans and their families. The LMSW is responsible for addressing resident and family complaints and assisting with long-term care considerations.
Responsibilities
- Meet with residents and family members upon admission and throughout their stay to assess and evaluate needs.
- Formulate and implement individualized action plans based on residents' strengths and needs.
- Participate in the development of interdisciplinary care plans and revise them as necessary.
- Review, investigate, and document all resident and family complaints.
- Provide psychological support and resources for residents' mental and physical health.
- Assist families with long-term care needs.
Requirements
- Must be licensed and currently registered by NYS as a Licensed Master Social Worker.
- Current State employee with permanent or contingent-permanent status as a Licensed Master Social Worker 1 or any NYS employee with one year of permanent or contingent-permanent service in a G-18 or higher title.
Nice-to-haves
- Social work experience in a skilled nursing facility working with geriatric residents.
