About the position

Responsibilities

  • Conduct psychosocial evaluations for patients and families.
  • Provide ongoing counseling and emotional support during the end-of-life journey.
  • Collaborate with the healthcare team to implement the plan of care.
  • Ensure that patients' unique needs are met with dignity and respect.
  • Participate in professional development opportunities and training.

Requirements

  • MS degree in social work from an accredited school of social work approved by the Council of Social Work Education.
  • Current state license as a social worker.
  • 2+ years of clinical work experience, preferably in healthcare or hospice.
  • Reliable transportation.
  • Ability to sit, stand, bend, move intermittently and lift at least 25lbs.

Nice-to-haves

  • Experience in hospice or palliative care settings.
  • Familiarity with mental health and counseling programs.
