Responsibilities

• Provide assessment, evaluation, and management of patients.
• Participate in the collection of patient data as needed.
• Determine nursing care needs based on patient data and health status.
• Communicate information effectively to the healthcare team and patient support system.
• Participate in the development of individualized care plans in collaboration with the registered nurse.
• Document patient care information in a timely manner.
• Identify nursing interventions and goals for patients and review them with the registered nurse.
• Engage in self-reflection and pursue professional development opportunities.

Requirements

• High School Diploma required.
• Graduate of an accredited Practical Nursing Program required.
• 1 year of relevant experience required.
• Applicable state LPN license required.
• Ability to complete general and department-specific skills validation and competency testing.
• Knowledge and skills necessary to modify care according to patient age-specific needs.
