About the position

The Licensed Practical Nurse (LPN) position at Haven Place Long Term Care involves providing essential resident care in accordance with the Practical Nurse Act of Pennsylvania. The role emphasizes collaboration with an interdisciplinary team to meet the physical, medical, and psychosocial needs of residents, ensuring a high standard of care in a skilled nursing and rehabilitation facility.

Responsibilities

• Provide resident care within the scope of the Practical Nurse Act of Pennsylvania.
• Assist the RN and interdisciplinary team in evaluating, planning, and implementing plans of care.
• Communicate effectively with residents and families about care concerns.
• Comply with all safety policies and procedures, including Infection Control Precautions.
• Observe and record resident's symptoms, condition, and reactions to medications and treatments, reporting results to the RN in charge.
• Oversee care given by Certified Nursing Assistants to ensure adequate and timely care is provided.
• Participate in resident care conferences and self-review as requested by the Unit Manager.
• Perform assigned work in a timely and productive manner, including giving injections, taking vital signs, and administering medication.

Requirements

• Completion of a Practical Nursing Program approved by the State Board of Nursing.
• Licensure as a Licensed Practical Nurse (LPN) in Pennsylvania.
• Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification required within 30 days of hire.

Nice-to-haves

• Long term care experience preferred.
