About the position

La Crescent Health Services is seeking a Licensed Practical Nurse (LPN) to join our team. This position involves working 12-hour shifts with base wages up to $35.50 per hour. We will cover the cost for your MN Border State License if you are licensed in a bordering state. As part of the North Shore Healthcare family, we are a leading provider of skilled nursing and rehabilitation services in the Upper Midwest, with 70 centers across Wisconsin, Minnesota, Michigan, and North Dakota. The LPN will work under the supervision of the RN Unit Manager or Director of Nursing (DON) and will be responsible for the care of assigned residents/patients during their shift. This includes coordinating, implementing, and evaluating the care plan, ensuring safety, and supervising nursing assistants. The LPN will adhere to all center policies and procedures and perform duties as defined by the State Nurse Practice Act.

Responsibilities

  • Provide nursing care to residents/patients as per the care plan.
  • Coordinate, implement, and evaluate the resident's/patient's plan of care.
  • Manage the environment to maintain resident/patient safety.
  • Supervise nursing assistants in their care activities.
  • Adhere to the standards of care for the area and follow all center policies and procedures.

Requirements

  • Licensed in the State as an LPN.
  • Current CPR certification.
  • Ability to use necessary supplies and equipment (feeding pumps, oxygen, treatment trays, medication cart, etc.).
  • Ability to calculate drug dosage accurately.
  • Meet all health requirements imposed by law.
  • Ability to relate positively and effectively with residents, families, community members, and other staff.
  • Ability to read, write, speak, and understand English; basic computer knowledge and aptitude to learn company applications.
