About the position
The Licensed Practical Nurse (LPN) - Home Health position is a full-time, direct-hire role focused on delivering compassionate, high-quality patient care in the comfort of patients' homes. The LPN will collaborate with healthcare professionals to implement care plans and ensure compliance with home health regulations.
Responsibilities
- Administer medications and monitor vital signs.
- Assist patients with daily living activities.
- Collaborate with physicians and registered nurses to implement care plans, including wound care and injections.
- Accurately document patient information in electronic health records (EHR).
- Educate patients and families on care plans and medication management.
- Ensure adherence to home health regulations and advocate for patient needs.
Requirements
- Current and unrestricted LPN license in the state of South Carolina.
- Minimum of 2 years of practical nursing experience, preferably in home health or a clinical setting.
- Basic Life Support (BLS) certification required.
- Strong clinical skills and proficiency in EHR systems.
- Excellent communication abilities.
- Compassionate, adaptable, detail-oriented, and comfortable working independently.
