About the position

Valley Health System is seeking a dedicated Licensed Practical Nurse II to join our VRE Home Health team. As a not-for-profit network of hospitals and healthcare services, we are committed to providing exceptional care to our community across Virginia, West Virginia, and Maryland. In this role, you will be responsible for delivering direct patient care in the comfort of patients' homes, working collaboratively with a multidisciplinary team that includes Registered Nurses (RNs), Physical Therapists (PTs), Occupational Therapists (OTs), speech therapists, social workers, and home health technicians. Your primary responsibilities will include assessing patients and implementing individualized care plans developed by the RN. You will provide one-on-one patient care, monitor changes in patient status, and communicate these changes to the RN Case Manager. The role requires proficiency in using technology, as you will utilize a laptop for documentation in an electronic health record system. The patient population you will serve is diverse, encompassing various ages, diagnoses, and levels of acuity, which presents unique clinical challenges and opportunities for meaningful impact in their lives. To ensure you are well-prepared for this role, you will undergo a comprehensive 6-10 week orientation with an experienced preceptor. This training will equip you with the necessary skills and knowledge to provide high-quality care independently. Additionally, you will have opportunities to engage in performance improvement initiatives, policy and procedure development, and research activities. Responsibilities

  • Provide direct patient care in the patient's home environment.
  • Assess patients and follow individualized care plans established by the RN.

  • Participate in research and development activities.

Requirements

  • Associates Nursing either LPN associate degree or certificate diploma required.
  • Must meet all requirements of an LPN I ladder to advance to LPN II.

Nice-to-haves

  • Previous home health experience preferred, but not required.
