About the position

The Licensed Practical Nurse (LPN), under the direction of the Practice Manager/Director, provides nursing care to patients and assists with patient education. Assists provider during the treatment and examination of patient, and administers procedures and treatments as directed by the provider. The LPN should be prepared to handle the triage of patient calls; including prescription refills in accordance to refill protocols. The LPN is able to maneuver in the patient electronic health record (EMR) and documents patient vitals, and other pertinent information appropriately. Customer service skills are essential in all aspects of this position.

Responsibilities

• Timely, accurate, and thorough completion of medical documentation
• Ability to give immunizations
• Appropriate documentation of patient information on intake
• Ordering of pertinent medical supplies and keeping inventory stocked
• Ability to utilize computer/computer software for pertinent patient documentation
• Adhere to policies/procedures for patient medications and testing
• Electronic communications with patients
• Review of patient information via huddles to comply with established PCMH measures
• Excellent communication (both written and verbal)
• Effectively coordinate patient care with pertinent referrals and/or follow-ups
• Meaningful use compliance to standards
• Timely management of patient needs/requests
• Maintain pertinent logs of work performed
• Adhere to clinic specific competencies
• May be required to lead the clinical team in PCMH measures

Requirements

• Associate's Degree or Graduate of a Technical School
• Kansas LPN License
• BCLS obtained through the American Heart Association
