About the position

The Licensed Practical Nurse (LPN) Case Management position at Community Interactions, Inc. is a full-time role focused on supporting individuals with intellectual, physical, and behavioral health disabilities. The LPN will assist these individuals in achieving their desired outcomes as outlined in their Annual Plan, perform general office duties, and conduct site visits to ensure medication administration accuracy and provide necessary training. This role requires both office and fieldwork, including visits to homes and other work sites, necessitating physical activity and travel to program sites and meetings.

Responsibilities

  • Conduct site visits to check medication administration accuracy.
  • Provide site/individual based training on diagnoses, medications, and procedures.
  • Deliver general training for all staff on various health-related topics.
  • Ensure all pertinent data from past medical history is recorded in the current lifetime medical history.
  • Capture all pertinent dates from medical appointment documentation and update lifetime medical history annually during planning meetings.
  • Establish contact with hospitals when a consumer is hospitalized and act as the primary point of contact.
  • Coordinate discharge plans with hospital staff and ensure adequate supports are in place post-discharge.
  • Attempt to obtain pertinent hospital records at the time of discharge.
  • Act as the primary point of contact for pharmacies regarding medication issues.
  • Attend planning meetings to provide necessary medical information.
  • Advocate for the individual's healthcare needs and maintain communication with the individual's team regarding health-related issues.
  • Be on call for assessment of medical issues as needed.
  • Review hospital discharge instructions and ensure changes are implemented for the health and safety of individuals.
  • Perform other duties as required by the Director of Incident Management and Quality Assurance.

Requirements

  • Must hold a Practical Nurse license in the state of Pennsylvania.
  • Minimum of three years of experience in the nursing field.
  • Experience working with individuals with disabilities is preferred.
