About the position
The Licensed Practical Nurse (LPN) position at StoryPoint Prospect involves providing essential nursing care in an assisted living and memory care environment. The role emphasizes leadership, management, and training of caregivers while ensuring high-quality care for residents. The LPN will be responsible for evaluating residents' health conditions and communicating effectively with the healthcare team, residents, and their families.
Responsibilities
- Lead, manage, and train your team of caregivers.
- Responsible for evaluating residents and reporting changes in condition to physicians.
- Utilize Electronic Health Record (EHR) and Electronic Medication Administration Record (EMAR).
- Communicate effectively with team members, residents, and families.
- Build strong relationships with residents and their families.
Requirements
- Licensed Practical Nurse (LPN) in the state of Kentucky.
- Assisted Living/Memory Care experience preferred.
- Prior supervisory experience preferred.
- Punctuality and professionalism are important factors.
- Passionate about working with the elderly.
