About the position

The Licensed Practical Nurse (LPN) position at BJC Home Care focuses on providing compassionate and continuous end-of-life care to hospice patients in their homes. The role involves working night shifts, specifically from 7 p.m. to 7:30 a.m., three days a week, with a rotating schedule that includes every other weekend and one major and one minor holiday per year. The LPN will be part of a multidisciplinary team dedicated to addressing the physical, emotional, and spiritual needs of terminally ill patients and their families.

Responsibilities

• Implements treatments and procedures within the scope of practice for the Licensed Practical Nurse.
• Provides patient teaching and discharge planning under the direction of the Registered Nurse.
• Participates in professional development and lifelong learning.
• Performs patient assessment and evaluation considering the patient's age and developmental needs.
• Assists in developing and updating the plan of care based on the patient's age and developmental needs.
• Implements the plan of care with regard to the patient's age and developmental needs.

Requirements

• High School Diploma or GED
• Valid Driver's License
• LPN - IV Certification
