About the position

The Licensed Practical Nurse (LPN) Referral Center at Duke University Health System is responsible for coordinating internal and external referrals for Duke HomeCare, Hospice, and Infusion Services. This role does not involve direct patient care but focuses on the initial intake of provider orders and ensuring comprehensive, individualized care is provided in collaboration with a multidisciplinary team.

Responsibilities

  • Coordinate internal and external referrals for Duke HomeCare, Hospice, and Infusion Services.
  • Hold responsibility for the initial intake of provider orders.
  • Participate in weekly team meetings and case conferences to facilitate appropriate services for patients.
  • Maintain up-to-date knowledge of Medicare/Medicaid regulations through in-service and workshop attendance.
  • Coordinate care plan activities with multidisciplinary team members as directed by the Nursing Case Manager.
  • Document care and services provided in accordance with DHCH policies and regulatory standards.
  • Provide timely, concise, and complete documentation of care.
  • Follow standard precautions and use Personal Protective Equipment (PPE) as required.
  • Observe patients' reactions to medications and report any adverse reactions to the supervising nurse or physician.
  • Communicate regularly with team members, caregivers, and physicians.
  • Implement agency policies, procedures, and objectives.
  • Maintain confidentiality of employee and patient information.
  • Exhibit an attitude that promotes harmony and goodwill in the workplace.
  • Participate in the Performance Improvement Program.
  • Promote quality, comprehensive services through a team approach.
  • Perform other duties as assigned.

Requirements

  • Graduation from an accredited Practical Nurse program is required.
  • Current State of North Carolina LPN licensure or compact license in a participating state is required.
  • One to two years of experience preferred.
  • Working knowledge of Medicare and Medicaid regulations is preferred.
  • Strong computer skills preferred.

Nice-to-haves

  • Knowledge of the scope of licensed practical nursing and ability to delegate to nursing support staff.
  • Considerable knowledge of patient care and treatment procedures.
  • Ability to communicate clearly with patients, families, and healthcare teams.
  • Competence in BLS and/or other specialized life support requirements.
