Traditional Home Health Care is hiring compassionate, dedicated, Licensed Practical Nurses to provide one-on-one care to our patients located in Luzerne County. This position offers a flexible schedule with both full-time and part-time hours available. The role includes responsibilities such as implementing and documenting the plan of care to ensure quality and continuity of care for patients, administering prescribed medication, treatments, and therapies, conducting patient assessments, and coordinating care. Additionally, nurses will report any changes in the client's condition to the Clinical Manager and physician, and engage in health promotion, teaching, and training of family members. Training and orientation will be provided, along with paid on-site training and annual education.
