About the position

The Licensed Practical Nurse (LPN) at the Department of Veterans Affairs is responsible for delivering direct patient care to adult through geriatric Veterans in an outpatient setting. The role involves implementing both basic and advanced LPN procedures as defined by the Illiana Health Care System's policies. The LPN will maintain ongoing training and education to support enhanced roles and will be accountable for recognizing and responding to changes in patients' conditions.

Responsibilities

  • Deliver direct patient care to adult through geriatric Veterans for both acute and chronic diseases in an outpatient setting.
  • Implement basic and advanced LPN procedures as defined by policies and procedures of the Illiana Health Care System.
  • Observe and report changes in patients' physical and emotional conditions using nursing process elements.
  • Perform health history screenings and follow through for acute/chronic medical and emotional conditions with minimal supervision.
  • Assist in planning, implementing, documenting, and evaluating patient care in the outpatient setting.
  • Recognize priorities and adjust patient care accordingly, especially in urgent and emergency situations.
  • Demonstrate clinical competence in performing core and practice-specific procedures according to the Competency Checklist.
  • Prepare, administer, and document actions taken for prescribed medications as permitted by policy/procedure.

Requirements

  • Graduate of a school of practical or vocational nursing approved by the appropriate State agency or accredited by the Accreditation Commission for Education in Nursing (ACEN).
  • Must possess a Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory, or Commonwealth of the United States, or District of Columbia.
  • Proficient in written and spoken English.
