About the position

The Licensed Practical Nurse (LPN) position at Adoration Home Health and Hospice involves providing essential nursing care under the supervision of a Registered Nurse. The role focuses on monitoring clients' health, administering medications, and ensuring effective communication with clients and their families. The LPN is responsible for maintaining the dignity and independence of clients while adhering to their care plans.

Responsibilities

  • Providing 1:1 care for clients following their plan of care while maintaining dignity and independence.
  • Monitoring clients' vital statistics and reporting abnormal findings to the Case Manager and Physician.
  • Administering medications, feedings, oxygen, and ostomy care as needed per the client's plan of care.
  • Completing routines established by other care professionals such as PT, OT, and Speech Therapists.
  • Ensuring infection control policies are maintained and reporting possible communicable diseases or infections.
  • Accurately documenting nursing actions and communication with family, case managers, and physicians.
  • Coordinating or ordering supplies, medications, and feedings to ensure adequate amounts are on hand.
  • Monitoring incidents involving individuals and ensuring incident reports are completed timely and accurately.
  • Ensuring all medical equipment is maintained and reporting faulty equipment for repair.
  • Performing other duties as assigned.

Requirements

  • LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice.
  • Valid driver's license, acceptable driving record, and proof of car insurance.
  • CPR professional certification.
  • Experience in administering medications.
  • 1+ years of nursing experience preferred.
  • Home health care experience preferred.
  • Ability to lift and carry items weighing up to 20 pounds.
  • Ability to bend, stoop, push, pull, sit, and walk for periods of time.
  • Must be able to communicate both verbally and in writing.
