About the position

Responsibilities
- Provide high-quality nursing care to residents.
- Collaborate with healthcare team to implement individualized treatment plans.
- Supervise and evaluate CNAs to ensure high standards of care.
- Oversee fall-risk and pressure ulcer prevention programs.
- Conduct thorough evaluations of residents' medical status upon admission and throughout treatment.
- Document changes in residents' conditions and adjust care plans as necessary.
Requirements
- Active license to practice as an LPN.
- Experience in long-term care, skilled nursing, or rehabilitation facilities is preferred.
