About the position

The Licensed Practical Nurse (LPN) at Caregiver, Inc. is responsible for overseeing the healthcare of consumers with intellectual and developmental disabilities, providing direct nursing services, and ensuring compliance with medical protocols. The role emphasizes promoting quality of life for consumers, supporting families, and participating in policy shaping related to disability services. The LPN will work under the supervision of the Clinical Services Manager and will be involved in various healthcare systems and training programs.

Responsibilities

  • Provides direct and individualized nursing care to a caseload of consumers.
  • Responsible for Wellness Coordination Services, including documentation and service provision as required by State regulations.
  • Ensures prescribed medications and treatments are available in the home for each consumer.
  • Develops, documents, implements, and evaluates health care protocols and nursing care as required by the Individual Support Plan (ISP).
  • Acts as a resource for agency LPNs as assigned.
  • Monitors vital signs, weight charts, immunization records, and health status of consumers.
  • Reviews the agency medication error report and makes recommendations to the Executive Council.
  • Manages the agency Psychotropic Medication Review Clinic as assigned.
  • Participates in Area Individual Support Team (IST) meetings, sharing knowledge and ideas among team members.
  • Records information concisely, accurately, and completely.
  • Ensures consumer's medical information chart is current.
  • Maintains a positive working relationship with family members and guardians of consumers.
  • Serves as the primary contact when a consumer is hospitalized, communicating between agency staff and hospital staff.
  • Assists in developing staff training curriculums, including training in detecting signs of illness or dysfunction.
  • Administers and reads TB skin tests as necessary for consumers and staff.
  • Ensures nursing assessments are completed per agency policy.
  • Serves as a liaison between health care allied service providers and Area Directors.
  • Maintains consumer medical records in compliance with HIPAA and other regulations.
  • Participates in the nursing on-call schedule.
  • Represents DSA, Inc. to trade organizations and state or regional committees.
  • Participates in routine supervisory reviews with the Clinical Services Manager.
  • Appoints a person to act on her/his behalf during absences.
  • Performs other duties as assigned.

Requirements

  • Degree from an accredited school of nursing.
  • Current state licensure as a licensed nurse.
  • Five years of clinical practice preferred.
  • BSN preferred.
  • One year of experience working with individuals with developmental disabilities or in a related field preferred.
  • Maintains credentials necessary to teach Core A/B courses.
  • Maintains certification necessary to administer and read tuberculosis skin tests.
  • Must be at least twenty-one (21) years of age.
  • Ability to communicate effectively, orally and in writing.
  • Ability to use common word-processing, spreadsheet, and communications software programs within a Windows environment.
  • Satisfactory completion of criminal background checks as required by federal and/or state laws.
  • Valid Indiana driver's license, a good driving record, and a current automobile liability insurance policy are required.
