About the position

The Licensed Practical Nurse (LPN) - Maternity Care Coordinator at the Northport VA Medical Center is responsible for providing comprehensive care to pregnant and postpartum veterans. This role involves coordinating multi-condition care, ensuring veterans understand their maternity benefits, and facilitating access to necessary health services. The LPN will work closely with healthcare teams to support the unique needs of veterans during and after pregnancy, while also being trained as a Lactation Consultant.

Responsibilities

• Ensure pregnant veterans understand the full range of VA maternity benefits and resources.
• Assist pregnant veterans in navigating the Community Care referral process.
• Screen pregnant veterans for targeted health and social needs, including mental health and wellness.
• Facilitate obtaining VA care for pregnant veterans as needed.
• Assist in transitioning pregnant veterans back to PACT or specialty care teams during the postpartum period.
• Provide lactation consulting as trained for the facility.

Requirements

• U.S. Citizenship is required.
• Graduation from a school of practical or vocational nursing approved by the appropriate State agency.
• Full, active, current, and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory, or Commonwealth of the U.S.
• Proficiency in written and spoken English.
