About the position

The Richland Hospital, Inc. is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for the Medical-Surgical/Special Care Unit (SCU). This position is primarily a twelve-hour night shift role, with some variations to day shifts, and includes a rotation of every third weekend and holiday. The LPN will play a crucial role in providing high-quality nursing care to patients, ensuring that their physical, psychological, and social needs are met. In this role, the LPN will be responsible for age-specific data collection and performing thorough observations of patients. They will assist the registered nurse in developing and implementing a comprehensive plan of care, providing direct patient care under the supervision of the RN. Key responsibilities include medication administration, care planning, intervention, and evaluation of treatment, as well as telemetry monitoring. The LPN will collaborate closely with the RN and other interdisciplinary team members to create individualized care plans that incorporate educational and discharge planning interventions. The LPN is expected to provide age-appropriate nursing care in accordance with the standards of Clinical Nursing Practice. Effective communication and collaboration with the direct patient care team are essential to prioritize workloads and achieve optimal patient care outcomes. The LPN will also promote a cohesive work environment and maintain professional interactions with all members of the patient care team, ensuring a supportive atmosphere for both patients and staff.

Responsibilities

  • Perform age-specific data collection and observations of patients' needs.
  • Assist the registered nurse in developing a plan of care.
  • Provide direct patient care under the supervision of the registered nurse.
  • Administer medications as prescribed.
  • Participate in care planning, implementation, intervention, and evaluation of treatment.
  • Monitor telemetry and report any significant changes to the RN.
  • Collaborate with the RN and interdisciplinary team to develop individualized care plans.
  • Engage with patients, families, and significant others in care planning and education.
  • Communicate effectively with the direct patient care team to prioritize workload.
  • Promote a cohesive and professional environment among the patient care team.

Requirements

  • Successful completion of an accredited practical nursing program.
  • Current licensure as a Licensed Practical Nurse in the state of Wisconsin.
  • BLS certification required or completion within 3 months of hire.
  • Previous nursing experience preferred.
