About the position

The PACT Licensed Practical Nurse (LPN) at the Department of Veterans Affairs is responsible for providing specialized nursing care to adult and geriatric patients with both acute and chronic health issues. The LPN collaborates with the Patient Aligned Care Team (PACT) to develop individualized health care and management plans for patients. This role requires adherence to nursing standards and practices, effective communication with patients and families, and the ability to recognize and respond to urgent care situations.

Responsibilities

• Administer nursing care to patients in Primary Care settings.
• Document nursing care and gather data for assessments on chronic diseases and preventive screenings.
• Administer medications and observe for therapeutic effects and adverse reactions.
• Take and document vital signs, height, and weight according to policy.
• Recognize urgent patient care situations and initiate appropriate emergency interventions.
• Perform EKGs and notify the RN/APN/MD of findings.
• Use motivational interviewing and health coaching to guide patients in self-management.
• Inventory supplies and medications, ensuring stock levels are appropriate.
• Complete clinical reminders and appropriate consults as indicated.
• Assist in the orientation and training of less experienced LPNs/LVNs.

Requirements

• U.S. Citizenship is required.
• Graduation from an approved school of practical or vocational nursing.
• Full, active, current, and unrestricted licensure as a Licensed Practical Nurse in a U.S. state or territory.
• Proficiency in written and spoken English.
• Ability to pass a pre-employment physical evaluation.
