About the position

The Licensed Practical Nurse (LPN) position at the Minot VA Clinic is part of the Patient Aligned Care Team (PACT) model, focusing on patient-centered care. The LPN will perform specialized nursing activities under the supervision of a Registered Nurse, utilizing their skills to independently manage a range of nursing care decisions. This role emphasizes the importance of creating personalized health plans for Veterans and requires effective communication and collaboration with healthcare teams.

Responsibilities

  • Initiates, performs, and completes assigned duties in providing care to variable patient populations.
  • Serves as a preceptor to orient and train less experienced LPNs and Nursing Assistants in acceptable standards of practice.
  • Carries out assigned patient care based on patients' conditions, using judgment in selecting appropriate procedures and treatments.
  • Observes, identifies, and responds to patients' needs for care, including medication and patient/family education.
  • Interacts appropriately with patients, families, and other healthcare personnel involved in patient care delivery.
  • Documents patients' responses to medications and treatments, reporting any changes in condition to the RN or provider.
  • Recognizes urgent or emergent patient care situations and initiates appropriate emergency interventions.

Requirements

  • Must be a U.S. Citizen.
  • Graduate of an approved school of practical or vocational nursing.
  • Full, active, current, and unrestricted licensure as an LPN in a State, Territory, or Commonwealth of the U.S.
  • Basic Life Support (BLS) certification required.
  • Proficient in written and spoken English.
