About the position
The Licensed Practical Nurse (LPN) in the Primary Care Department of Veterans Affairs is responsible for providing high-quality nursing care to a diverse veteran population. This role requires independent technical judgment and critical thinking skills to manage complex patient care assignments. The LPN will collaborate with a Patient Aligned Care Team (PACT) to ensure timely access to care and will serve as a resource for specialized procedures. The position is full-time and based in Murfreesboro, TN, with no travel required.
Responsibilities
- Provide direct patient care activities, including managing patient care problems and deviations.
- Assess, record, and report vital signs, neurological signs, weight, and blood glucose levels.
- Triaging and prioritizing patient needs as routine, urgent, or emergent in the clinic setting.
- Check patients into the clinic, interview patients and/or significant others, and document intake data as per policy.
- Serve as a resource for complex/specialized procedures such as suture removal, wound care, and blood draws.
- Assist with diagnostic procedures, including preparing the patient and monitoring their condition.
- Collaborate with the PACT team and other interdisciplinary services in the development and evaluation of patient care.
- Follow up on abnormal results with the provider and educate patients/families on disease management.
- Coordinate clinical work assignments with the RN to ensure patient care needs are met.
- Assist the Nurse Manager with planning and organizing specialized unit projects.
Requirements
- U.S. Citizenship is required.
- Graduation from a school of practical or vocational nursing accredited by the appropriate state agency.
- Active, full, current, and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse in a state or territory of the U.S.
- Proficient in spoken and written English.
- Minimum of 3 years of LPN experience, with at least one year at the GS-6 level or equivalent.
Nice-to-haves
- Experience in primary care settings.
- Ability to work with geriatric patients and those with complex psychiatric needs.
- Leadership experience within a healthcare team.
