About the position
The Licensed Practical Nurse (LPN) position involves providing direct nursing care to patients in their homes, ensuring high-quality patient care in compliance with state laws and agency policies. The role offers flexibility, opportunities for career growth, and the chance to build meaningful nurse-patient relationships while working under the supervision of a Registered Nurse.
Responsibilities
- Provide direct nursing care as prescribed by the physician to acutely and chronically ill patients in their homes.
- Document patient visits thoroughly in the electronic medical record as per regulatory and payer requirements.
- Participate in the agency's care coordination process, reporting patients' needs and changes in condition.
- Ensure confidentiality of patient health information according to agency policy.
- Coordinate with other members of the healthcare team to provide the highest quality of patient care.
Requirements
- Must have current LPN licensure in the state of practice.
- A minimum of one year of acute care experience preferred; previous home care experience preferred.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification required.
- Ability to relate to and treat acutely ill/chronically ill patients and their families.
