About the position

The Licensed Practical Nurse (LPN) position at Willow Valley Communities involves providing skilled and personal care to residents in a supportive environment. LPNs will work in a fast-paced setting, ensuring high-quality nursing care while maintaining a regular schedule that promotes work/life balance. The role includes supervising nursing staff, administering medications, and assisting with daily living activities, all while adhering to nursing standards and protocols.

Responsibilities

  • Supervises and coordinates personnel delivery of nursing care on assigned shift in skilled care.
  • Accurately transcribes and carries out physicians' orders.
  • Administers prescribed medications and treatments in accordance with approved nursing techniques.
  • Assists in development and implementation of Resident Care Plan; updates Care Plan as needed.
  • Responds to life-saving situations based upon nursing standards, policies, procedures and protocol.
  • Collects samples such as urine, blood and sputum from residents for testing and performs routine laboratory tests on samples.
  • Assists Residents at meal times and monitors dining room process.
  • Records food and fluid intake and output.
  • Assists in Resident care, including bathing, toileting, and other ADLs.
  • Responds to Resident call bells in a timely manner.
  • Ensures consistency among Supportive Living Communities in all operations, policies and procedures.
  • Administers treatments and medication as specified in MAR, TAR, PRN sheets and follow properly.
  • Assembles and uses equipment including catheters, tracheotomy tubes and oxygen suppliers.
  • Assists with providing guidance and assistance to Residents as needed with ADLs.
  • Reviews 24 hour report and general notes in PCC from the preceding shift.
  • Follows through on unfinished orders/tasks from shift prior.
  • Assures Resident care is completed by assignments.
  • Documents changes in Resident care and condition.
  • Maintains calendar of Resident appointments and assures that transportation of Residents to appointment.
  • Assesses Residents' condition and informs physician, family/responsible party of any changes and plan of action/treatment, as needed.
  • Receives orders from physicians and brackets order with time, date and initials. Transfers order to PCC.
  • Notifies pharmacy of changes in medication orders or need for refills, initial and date order changes.
  • Complies with Department of Human Services regulations and follows the WVC policies and procedures.
  • Completes lab requests per physician orders with all pertinent information.
  • Assures that Residents are seen by a physician annually for medical evaluation or when change in Resident's condition and that physician completes a DME.
  • Assures a RASP is completed with each DME.
  • Assures that physicians complete necessary paperwork.

Requirements

  • PA Practical Nurse License required.
