About the position

The Licensed Practical Nurse (LPN) in Telemetry provides essential therapeutic nursing care under the supervision of a Registered Nurse (RN). This role focuses on delivering routine and standardized care to patients, ensuring their safety and well-being while collaborating with the healthcare team to achieve optimal patient outcomes. The LPN is responsible for administering medications, documenting patient care activities, and supporting discharge planning, all while adhering to established nursing standards and guidelines.

Responsibilities

  • Provide direct patient care utilizing the nursing process under the supervision of the RN.
  • Review diagnostic test results and report abnormal findings to the RN and healthcare team.
  • Administer prescribed medications, therapies, and treatments according to nursing standards.
  • Accurately document patient care activities and maintain medical records in compliance with standards.
  • Implement components of the care plan within the LPN scope of practice to promote desired outcomes.
  • Assist with discharge planning and patient education under the direction of the RN.
  • Evaluate and maintain a safe environment for patients and healthcare team members.
  • Provide compassionate nursing care while ensuring patient privacy and confidentiality.
  • Collaborate with the RN to execute the patient's plan of care effectively.
  • Assist healthcare team members with data collection, assessment, and planning for patient care.
  • Advocate for patients' psychological, ethical, spiritual, and cultural needs in collaboration with the healthcare team.
  • Participate actively in unit teams and committees, anticipating needs and assisting others as required.
  • Support and implement outcomes from team initiatives and keep informed of unit activities.
  • Assist in the orientation of new employees under the supervision of the RN as needed.
  • Demonstrate knowledge of quality improvement initiatives and participate actively in them.
  • Complete other assigned duties as required.

Requirements

  • Graduate of an accredited LPN program.
  • Licensed as an LPN in the state of Louisiana or possess a work permit.
  • BLS certification required.
  • Proficient in English with strong verbal and written communication skills.
  • Computer skills necessary for documentation and patient care.
