About the position

The Licensed Practical Nurse (LPN) position at the VA Connecticut Healthcare System is responsible for assisting registered nurses in providing care to patients with acute and rapidly changing physical and emotional needs. The role involves administering medications, collecting data, assessing vital signs, and implementing care plans while ensuring patient safety and comfort. This position requires travel to other VA locations for training and coverage as needed.

Responsibilities

  • Administers and records medications and treatments.
  • Collects data and observations to determine patient's needs.
  • Accurately assesses, reports, and records vital signs.
  • Evaluates, adjusts, and carries out a developed plan of care.
  • Recognizes and quickly determines emergency situations and makes appropriate, timely interventions.
  • Promotes and encourages patients to participate in the therapeutic plan through emotional support.
  • Provides assistance for diagnostic and therapeutic procedures by preparing the patient, assisting with the procedure, and monitoring the patient's condition.
  • Performs glucometer testing, urine collection, nasal swabs, and phlebotomy duties.
  • Identifies and reports abnormal test results.
  • Functions as a chaperone and as a member of the multidisciplinary team.
  • Charts according to VA guidelines and documents nursing care administered with patient's response.

Requirements

  • U.S. Citizenship is required.
  • Graduation from a school of practical or vocational nursing approved by the appropriate State agency.
  • Full, active, current, and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory, or Commonwealth of the U.S.
  • Proficiency in written and spoken English.

Nice-to-haves

  • One year of Practical Nurse experience in a Primary Care and outpatient setting.
